Set Defaults and Hide Labels Area

I’m using this form for donors to schedule furniture pickups. http://zabsplace.org/donate/schedule-a-pickup/

Is there a way to hide the labels and drop down areas and set a default so it automatically selects and submita certain options?

I’d like it to choose:
~ Location: Donor
~ Service: Furniture Donation Pickup
~ Worker: ZABS Employee

I don’t want them to have to choose or even see the dropdowns. If I just hide it with css it won’t let them have access to the calendar because they haven’t selected the service they’re trying to book.
